We are excited to share our 2023 Annual Report: Celebrating 35 Years of Services. 

In 2023, the Council marked a remarkable 35 years of dedicated service, driven by the mission to facilitate access to primary health care services for residents of suburban Cook County facing financial barriers.

We’re grateful for the State of Illinois and the dedicated supporters who fuel our mission. As societal disparities emerged, our unique healthcare model shone brighter, bridging gaps for those underserved. Joined by local partners and Cook County Public Health, we tackled these issues head-on.

Our amazing medical providers are at the core of our success, making it possible to extend a helping hand to those in need. Your support resonates deeply in the lives we touch.
