We are excited to share our 2022 Annual Report: Community Solutions for Health Care Equity. 

In 2022, healthcare was a rollercoaster of challenges and moments that amazed us. Despite ongoing, unpredictable pandemic circumstances, Access to Care remained steadfast. Collaborating with Cook County, we boosted vaccinations and supported members through unexpected times.

We’re grateful for the State of Illinois and the dedicated supporters who fuel our mission. As societal disparities emerged, our unique healthcare model shone brighter, bridging gaps for those underserved. Joined by local partners and Cook County Public Health, we tackled these issues head-on.

Our amazing medical providers are at the core of our success, making it possible to extend a helping hand to those in need. Your support resonates deeply in the lives we touch.
