Your Guide to Access to Care
All Access to Care patients receive "Your Guide to Access to Care" with their ID card. You must read the Guide thoroughly. The program does not cover hospitalization or treatments ordered by any doctor other than your Access to Care physician. If a patient does not follow the guidelines for the program, he/she will be responsible for the charges.
Access to Care is a not-for-profit primary health care program. It is not an insurance company. Health insurance is more comprehensive and covers treatments not covered by Access to Care. Health insurance also requires payment of monthly premiums.
The purpose of Access to Care is to provide a delivery system of primary health care services for residents of suburban Cook County and northwest Chicago who have no insurance and might not receive care for financial reasons.
You are responsible for procedures performed at any facility not listed in Your Guide. If your Access to Care doctor orders tests - you must be sure to go to a listed x-ray or lab facility. Do not rely on the doctor to know where you should go for tests. If you have questions about whether a particular test will be covered, call Access to Care BEFORE proceeding. It is your responsibility to know the location of an Access to Care contracted facility. If the test is not covered under Access to Care, arrangements can be
made at a County facility to get the procedure performed at a reduced cost.
Reading "Your Guide to Access to Care" and following the instructions will prevent you from being responsible for unnecessary medical bills.
