If you live in suburban Cook County or northwest Chicago and can’t afford to see a doctor, Access to Care can help! Access to Care provides affordable primary health care to low income uninsured residents of suburban Cook County and northwest Chicago. This unique program allows patients to see a private primary care physician in their local community for $5.00 per visit. Other services include prescription medication for $10, $20 or $30; lab tests and x-ray services for $5.00. Financial restrictions apply. Proof of address and proof of income are required.
